Welcome to NH Healthy Families! We’re glad you’re a member. You can count on us to provide the resources you need to care for yourself and your family.
It’s time to take control of your health – and this is the place to start. Your new health insurance plan offers comprehensive medical care and benefits. It also includes valuable programs, educational tools and support.
This page will help you understand your health plan. It has all the details you need to help manage your health. You can also find tools to take action. Also, by being a Health Hero you can earn dollar rewards* just for completing healthy activities.
You will get a Welcome Packet from NH Healthy Families in the mail – once we are notified that you are a new member of our plan. Your Welcome Packet will contain your Member ID card, Health Risk Assessment (HRA) form to complete with your PCP and more! If you have questions about your Welcome Packet – call Member Services at 1-866-769-3085 (TDD/TTY 1-855-742-0123). Your next steps with your new plan:
- Create an online account. You can manage your health benefits, change your primary care provider, contact a plan representative, view claims and more.
- Complete your Welcome Call with a Member Services Representative to verify your PCP and PCP appointment scheduling within the first 90 days from enrollment and earn $30* in My Health Pays® rewards.
- Complete your Health Risk Assessment (HRA) (PDF) and return it to your PCP. If you need help completing the form, call us at 1-866-769-3085. You can earn $10* by working with your PCP** to complete your HRA.
- Complete your Authorization to Use and Disclose Health Information form (PDF) online or in your Welcome Kit.
- If pregnant, complete a Notification of Pregnancy form (PDF). This is available on your online account.
- Choose a provider. You can view the providers available to you on our online directory. Choose or change your provider through your online account.
*Some restrictions and limitations apply. Each member can earn up to $250 in cash and non-cash goods and services through June 30 each year.
**The PCP must submit a claim for the HRA completion to qualify for the reward.
